  • No discrimination in insurance coverage based on age, sex, sexual orientation, marital status, or gender identity

To be eligible for IVF coverage, the insured individual must have a diagnosis of infertility. Infertility is defined as a disease or condition characterized by the inability to impregnate another person or to conceive after 12 months of regular, unprotected sexual intercourse, or after six months for women who are 35 years of age or older.
